New Patient Registrationregister_now

All patients within the practice boundary are welcome to apply to join our practice list. You will be asked to complete registration and medical questionnaire forms and to provide some form of identification. An appointment will then be offered to attend for a health check. 

Unfortunately, we are not always able to accept patients living outside of this defined area. All current patients who move out of our practice area, unless discussed with their registered GP, may be expected to change practices.  If you have difficulty or need assistance in locating other primary care medical services you can contact the North East Primary Care Services Agency.

As a patient you also have the right to express a preference to which GP you see.  As a practice we will endeavour to comply with your request, however this may not be possible at all times.

When registering, it is useful (although not currently essential) if you have your NHS number. This number is unique to you and can help find your records more easily and also helps to improve safety between health agencies involved in your care.

Medical treatment is available from the date of registration. Please contact reception for further information.
